Concerns and hopes in the eyes of Parkinson's disease experts

Release date: 2016-04-27

Parkinson's disease is a common disease in middle-aged and elderly people, but because the cause of Parkinson's disease has not been clear so far, there are many treatment errors, and patients often miss the best treatment opportunity. In fact, Parkinson's disease can be treated and controlled, as long as you choose a reasonable treatment, you can effectively control the condition. On the occasion of the "World Parkinson's Disease Day" on April 11, we invited Professor Hu Xiaowu, a neurosurgery at Changhai Hospital affiliated to the Second Military Medical University, to talk about how patients should treat Parkinson's disease scientifically.

Most worried about the misunderstanding of patients with Parkinson's disease

Parkinson's disease is a degenerative disease that occurs with age, with a prevalence of 1.7% in people over 65 years of age. At present, the study is thought to be caused by degeneration and necrosis of dopaminergic neurons in the human brain and decreased secretion of dopaminergic neurotransmitters, which are characterized by a slow progression of life and are still incurable.

Professor Hu Xiaowu told us that patients with Parkinson's disease often fall into two extremes in the face of treatment. Some patients are reluctant to take medicine in time, subjectively believe that delaying medication can delay drug resistance. They are concerned that if there is drug resistance in early medication, there will be no other treatments in the later stages. In fact, in 2014, China released the third edition of the “Guidelines for the Treatment of Parkinson's Disease in China”. Compared with the first two editions, the most critical change is to emphasize “early detection and early treatment”. Another extreme patient is to be quick and desperate to take medicine. As long as he feels his symptoms are aggravated, he will add his own medicine. At first, his symptoms of stiffness and delay will improve, but he will soon become motionless. It is accompanied by side effects such as irritability and hallucinations. Leading to long-term treatment is getting worse and worse, and even an unsustainable situation will emerge.

Most hope that patients can find a treatment that suits them.

Professor Hu Xiaowu emphasized that Parkinson's disease has different treatment methods in different stages. Patients should not expect to see an outpatient clinic, and they will be able to set the medicine for the rest of their lives. The doctor will make different treatment plans according to the different periods of the disease, and the patients should go to the clinic at least once a quarter. If there is no change in the condition, the original medicine will be taken. If there is a new change, it will be adjusted. Once the drug-induced dyskinesia complications and drug efficacy decrease, patients who have not improved significantly after drug adjustment should consider surgery. .

Patients with Parkinson's disease will have a better time for drug treatment after the onset of illness. People call it the "honeymoon period." It refers to the use of lesser and smaller doses of drugs in the early or early years of Parkinson's disease, which can achieve satisfactory and sustained results. Usually, the quality of life of patients during the honeymoon period is relatively high, normal work, Life is not affected. After about 3-5 years, more than half of the patients will have incomprehensible motor complications such as dyskinesia and exercise fluctuations.

Many patients will still try to change their medications after the honeymoon period disappears. They are expected to re-create honeymoons with new drugs, until every drug fails, it will drag the unselectable surgery. treatment. This practice not only affected the treatment of their own life, but also delayed the timing of treatment. Professor Hu Xiaowu stressed that once the honeymoon period of the drug disappears, surgery should be considered immediately.

The surgery here refers to the treatment of brain pacemakers. Clinically, brain pacemakers can significantly improve the symptoms of stiffness, tremors, and exercise retardation in Parkinson's patients, reduce gait, balance, and dystonia, and control most of the central axis symptoms such as standing, opening, turning, and turning over; It can effectively reduce the dosage and side effects of taking the medicine. This is a surgery that uses minimally invasive neurosurgery to implant an electrode into a predetermined target area of the brain and connect it to a neurostimulator through a connecting wire, which is typically implanted under the chest skin, its size and heart. Similar to the pacemaker, the electrodes implanted in the brain emit a discharge pulse to control the relevant nerve nuclei of the movement, regulate abnormal neuroelectric activity, and achieve the purpose of reducing and controlling the symptoms of Parkinson's disease. This technology is quite mature in Europe and the United States. Many studies have also shown that the effect is obvious, many patients can significantly reduce the amount of medication after surgery, and the activity capacity is significantly improved. As one of the earliest hospitals for brain pacemaker surgery in the country, Changhai Hospital has accumulated 16 years of experience and its surgical technique is very mature.

Most looking forward to seeing the patient share after returning to normal life

In addition to standardized medical treatment, a positive and optimistic attitude towards life is also an essential part of the treatment of Parkinson's disease. Professor Hu Xiaowu suggested that patients and their families should establish a positive attitude, and believe that Parkinson's disease can be treated without fear. Among the patients with Parkinson's disease he treated, there is one Mr. Chen (a pseudonym) who used to be a semi-professional photographer. Parkinson's disease not only took away his hobbies, but even the camera was a luxury. . However, Mr. Chen did not give up treatment. He was optimistic about the disease and actively took the brain pacemaker surgery. Mr. Chen returned to the United States for a while and took many beautiful and clear photos and shared them with others. Parkinson's disease patients watch. Professor Hu Xiaowu emphasized that this kind of mutual encouragement between the groups and the transmission of positive energy can not only help to improve the symptoms, but also to solve the patient's mood, which is very worthy of promotion.

On the occasion of the arrival of "World Parkinson's Disease Day", under the joint support of the Parkinson's Disease and Movement Disorders Group of the Chinese Medical Association Neurology Branch and the Neurology Specialist Branch of the Chinese Medical Association, Parkinson's Disease and Movement Disorders Committee "Bright life beautiful life" 2016 Parkinson's disease patient essay contest officially launched. Professor Hu Xiaowu also expressed the hope that through the public interest form of the photography essay contest, the living conditions of patients or their families of Parkinson's disease will be displayed, causing resonance between patients and their families, causing social concern and promoting the popularization of science and disease education.
