The positive role of Freedom EVO in the diagnosis of vitamin B for diagnosis

Mass spectrometry is rapidly growing into the preferred quantitative method for the determination of trace elements (such as vitamins B1, B6, B12, and D), steroids, hormones, and many other drugs. For clinical laboratories, the bottleneck of mass spectrometry is not the analysis itself, but how to pretreat the sample and remove proteins and other biological molecules that may interfere with the results.

Over the past 30 years, people's understanding of the role of trace elements in maintaining the health of an aging population has become more and more profound. But only in the past decade or so, a reliable experimental method that can effectively detect the lack of trace elements in diet or metabolism becomes the norm. The deepening of understanding and the expansion of effective testing methods have led to a surge in demand for clinical laboratories from major or auxiliary institutions, which requires changes in the way the clinical laboratory changes its work to cope with the growing workload.

Vitamins B1 (thiamine) and B6 (pyridoxine) are responsible for extracting energy from glucose and fat, respectively, while participating in many other metabolic pathways. There has also been a significant increase in the demand for detection of these two vitamins. The Noeldwest Ziekenhuisgroep (Northwest Clinics) Alkmaar's clinical chemistry department in the Netherlands has to test more than 19,000 such metabolites a year. “As early as more than 20 years ago, we started to quantify vitamins B1 and B6 on HPLC systems, but this experiment involves a derivatization step that requires long-term follow-up analysis. In addition, the experiment requires a large number of manual loading techniques. Therefore, it takes a lot of time to do a high degree of re-reading work. In view of this, we have recently begun to seek ways to automate sample preparation, in order to improve throughput, reduce human error, and eliminate sample errors. We looked at a number of systems that could automate this type of sample preparation, and I was impressed with the display of the Freedom EVO® platform in Amsterdam. Our goal was to fully automate LC-MS sample preparation, and we purchased a Freedom EVO 150 workstation based on Tecan's recommendations.

Our lab's Freedom EVO began trials in the summer of 2016 and was optimized for 96-well plate applications. A reaction plate containing the patient's dissolved whole blood sample, a calibrator, and a quality control sample were placed on the instrument rack, and trichloroacetic acid (TCA) was added to each well to precipitate plasma proteins. The assay plates were then sealed and centrifuged in a centrifuge and the supernatant of each well was transferred to a new 96-well plate using a Tecan system. This test plate, which can be directly analyzed, was then resealed and transferred to a separate UHPLC-MSMS system. Edwin added: "Although we have not used the Tecan platform before, once you are familiar with Freedom EVO, you will find this system very easy to use. We have now carried out a number of test protocols on this instrument, and the repeatability of the automation has improved a lot compared to our previous pipetting procedures. By using hand-held barcode scanners and barcode-labeled microplates, we were also able to improve traceability – which is undoubtedly important for diagnostic laboratories.

Most importantly, Freedom EVO's automated sample preparation halved the time required to process an equivalent sample. It allows us not only to avoid the repeated pipetting work of the employees' samples, but also frees the human resources to perform other work tasks. Edwin concludes.
